Diabetes Education

Presentation for diabetes education, geared towards ambulatory care management, including risk for diabetes, basic lab values, signs and symptoms of hyperglycemia and hypoglycemia, proper nutrition, benefits of physical activity, and steps for self-maintenance. Education is also provided on medication, such as step therapy for proper dosing, side effects, and demonstrations on injectable medications.

1. Upon completion of this class, the learner will be able to explain the process of diagnosing diabetes.
2. Will have the knowledge necessary to teach their patients the common symptoms and complications of Type 2 Diabetes.
3. Will be aware of the documentation standards including utilizing care plans and SMART goals to achieve best possible outcomes for the patient.
4. Will have an introduction to oral and injectable diabetes medications.
5. Will have the knowledge and resources to teach patient skills including monitoring blood sugars and self-injections.
6. Will have the knowledge and resources to teach patients diabetes basics including nutrition and meal planning, carb counting and benefits of physical activity.
7. Will be knowledgeable about routine health maintenance for the diabetic patient.
8. Will have an introduction to insurance coverage basics and financial assistance resources.

Corewell Health West is approved as a provider of nursing continuing professional development by the Wisconsin Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.
