1. Standardized Communication Tools: Introduce tools like SBAR (Situation, Background, Assessment, Recommendation) to ensure clear and consistent communication across all team members.
2. Structured Handoff Processes: Develop and enforce standardized handoff procedures to ensure thorough and accurate transfer of patient information.
3. Interdisciplinary Team Training: Conduct regular training sessions and simulations that emphasize teamwork and collaborative problem-solving, breaking down silos and promoting a unified approach to patient care.
